Medical Schools

Consultation to US and Canadian medical schools on issues related to curricular design and LCME (Liaison Committee on Medical Education) accreditation. The Institute offers consultation to medical schools regarding administration and organization; curriculum design and evaluation; continuous quality improvement; faculty affairs; and student affairs. Consultation is available for medical schools preparing for an LCME site visit; medical schools on probationary, preliminary or provisional status; and applicant and candidate medical schools.  The Institute’s Medical Director has been involved in curricular reform and has served as Director of Medical Student Education in Psychiatry.

Psychiatry Residency Training Programs

Consultation to Psychiatry Residency Training Programs on issues related to ACGME (Accreditation Council for Graduate Medical Education) accreditation. The Institute offers consultation for Psychiatry Training Programs in achieving and maintaining compliance with requirements of the NAS (Next Accreditation System of ACGME) including Milestones and CLER (Clinical Learning Environment Review).  As a faculty member, TCI’s Medical Director facilitated, alongside the former Program Director, the emergence of a Psychiatry Training Program from ACGME probation.

Violence, Seclusion & Restraint Prevention/Trauma-Informed Care

Consultation to psychiatric hospitals, correctional institutions and other human services agencies on trauma-informed care (TIC) implementation with the goal of achieving sustainable organizational culture change leading to the prevention of violence and injury and the creation of coercion, seclusion and restraint free environments.  The Institute’s Medical Director has experience leading the implementation of TIC at a 300-bed forensic psychiatric hospital that resulted in a reduction in injuries and a 98% reduction in the use of restraints (from 230 episodes in year one to 5 episodes four years later). The Institute provides across discipline workforce training; on-site consultation with leadership, staff and treatment teams to assess readiness and barriers to change; review and revision of policies and procedures; on-site review of physical plant to review safety features and signage with a TIC lens; critical incident reviews (consultation/reviews of assaults, complex presentations, seclusion/restraint episodes, seclusion and restraint-related deaths and other critical incidents to assist with root cause analyses); and performance improvement/data monitoring consultation.

Psychiatric Hospitals and Correctional Facilities

Consultation to psychiatric hospitals, correctional facilities and substance abuse treatment programs regarding standards of care and safety for both individuals served and staff and related to achieving and maintaining compliance with CRIPA (Civil Rights of Institutionalized Persons Act) provisions and achieving and maintaining Joint Commission and CMS accreditation.   Consultation is available for institutions under review by or under settlement agreements with the US DOJ (US Department of Justice).  TCI’s Medical Director has experience working in a maximum security correctional facility and in a 300-bed forensic psychiatric hospital, both of which were under master settlement agreements with DOJ for CRIPA violations.   At the forensic psychiatric hospital, she led initiatives that achieved compliance with areas of the Agreement and that resulted in a reduction in injuries and a 98% reduction in the use of restraints (from 230 episodes in year one to 5 episodes four years later).

Government Agencies and Non-Governmental Organizations (NGOs)

Consultation to government agencies (DOJ, SAMHSA and others) and NGOs, The Joint Commission and other accreditation agencies on best practices, advocacy, policy and program development regarding mental health, substance abuse, trauma, violence and related areas.  TCI is available for consultation  to the US Department of Justice (DOJ) regarding institutional compliance with CRIPA provisions.  The Institute’s Medical Director has experience working in a maximum security correctional facility and a 300-bed forensic psychiatric hospital, both of which were under settlement agreements with the DOJ for CRIPA violations.  For five years TCI’s Medical Director also served as a consultant for SAMHSA’s National Center for Trauma-Informed Care, training across the country, across systems of care and across disciplines on TIC implementation and violence, seclusion and restraint prevention.
